About the Role:

As a Social Worker within the Neighbourhood Adults Social Work Team, you will play a vital role in supporting adults in the Liverpool community to live independently, with dignity, and have access to the services they need. You will work closely with individuals who may have complex needs, providing assessments, care planning, safeguarding, and multi-disciplinary collaboration to ensure the best possible outcomes.

Key Responsibilities:

* Conduct assessments of adults in need, considering their physical, emotional, and social well-being.

* Develop and implement personalised care plans that promote independence and support service users' rights and choices.

* Provide safeguarding interventions and manage cases in line with relevant policies and procedures.

* Collaborate with health professionals, community services, and families to ensure holistic care for individuals.

* Ensure compliance with statutory requirements and keep detailed, accurate records of casework.

* Advocate for vulnerable adults and their carers, promoting their rights and access to appropriate services.

* Engage with community resources to strengthen social networks and reduce isolation for service users.

Requirements:

* Qualified Social Worker (Degree in Social Work or equivalent).

* Registration with Social Work England.

* Experience in working with adults, particularly those with complex needs, physical disabilities, learning disabilities, or mental health issues.

* Strong knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and safeguarding protocols.

* Excellent communication, assessment, and decision-making skills.

* Ability to work independently and as part of a multi-disciplinary team.
