Job Overview We are seeking a Band 6 Occupational Therapist to join the Urgent Therapy Team in Central & South Liverpool. This role involves rapid assessments and interventions to prevent hospital admissions and facilitate timely discharges for patients with complex needs.
Key Responsibilities - Provide urgent occupational therapy interventions in both community and hospital settings.
- Conduct rapid assessments to support hospital avoidance and early discharge.
- Work collaboratively with multidisciplinary teams, including physiotherapists and social care professionals.
- Develop individualized care plans to improve patient independence and well-being.
- Maintain accurate clinical records in line with NHS guidelines.
Key Requirements - HCPC-registered Occupational Therapist with Band 6 experience.
- Experience in urgent care, hospital discharge, or community rehabilitation.
- Strong clinical reasoning and decision-making skills.
- Ability to work autonomously in a fast-paced environment.
- Car driver with access to a vehicle for community visits (preferred).
