About Us:

The Richmond Mental Health Social Care team within the London Borough of Wandsworth is seeking a passionate and experienced Social Care Assessor to join our dynamic team. We work collaboratively with adults aged 18-75 who are experiencing severe and enduring mental health conditions to support them in achieving their social care goals. Our team operates independently from the clinical community mental health teams, focusing solely on social care needs.

About the Role:

The successful candidate will be responsible for:

  • Conducting comprehensive Care Act assessments and reviews.
  • Undertaking Mental Capacity assessments as required.
  • Carrying out Carers assessments to ensure their needs are met.
  • Employing a strength-based approach to support service users in achieving their goals.
  • Participating in the Duty rota, providing frontline support to service users and their families.
  • Working effectively as part of a multidisciplinary team.

Essential Skills and Experience:

  • Proven experience in holding a caseload within a busy adult social care team.
  • Strong understanding of the Care Act legislation and its application.
  • Experience in conducting Mental Capacity assessments.
  • Knowledge of Carers assessments and support.
  • Experience in strength-based practice.
  • Excellent communication, interpersonal, and report-writing skills.
  • Ability to work independently and as part of a team.
  • IT proficiency, including the use of social care databases.

Desirable Skills and Experience:

  • Experience of working with adults with severe and enduring mental health conditions.
  • Knowledge of relevant social care legislation and best practice.
