• Band 6 Reablement Occupational Therapist

• Full or part time

• We are looking for a dynamic, experienced OT that will be able to compliment the services we provide.

  You will be working with OT, PT’s, TAPS (Therapy Assistant Practitioners) & RTW’s (Rehab Therapy Workers).

  The services we provide are:-

  • D2A (discharge to assess) where we facilitate discharges from acute and community hospitals.
  • Admission Avoidance (with 48 hours) to prevent admission to hospitals.
  • Planned therapy.

  We are a fast-paced service with a short length of stay and our overall aim is to provide a period of assessment to establish rehab goals and /or assess long term care needs. To accomplish this, we have some in house capacity for functional daily rehab visits, but we also can work alongside the supporting services when external care is required.
