Job Title: SW - CSWT - Isle of Sheppey

Location: Isle of Sheppey, Kent
Pay Rate: Up to £38 p/h

Job Description:

We are currently seeking a dedicated Social Worker to join our Community Social Work Team (CSWT) based on the Isle of Sheppey in Kent. This is an excellent opportunity for a skilled professional to make a real difference in the community, working with vulnerable individuals and families to deliver high-quality social care and support.

Key Responsibilities:

Conduct comprehensive assessments of individuals and families to identify needs and risks.

Develop and implement tailored care plans to support individuals in achieving their desired outcomes.

Work closely with a range of agencies and professionals to ensure a coordinated approach to care.

*

Ensure safeguarding and risk management procedures are adhered to.

*

Provide ongoing support, guidance, and advocacy for service users.

*

Maintain detailed case records and contribute to reviews and assessments as necessary.

Qualifications & Requirements:

*

Essential: Social Work qualification (e.g., Degree in Social Work, Postgraduate Diploma in Social Work).

*

Essential: Registration with Social Work England (SWE).

*

Proven experience in a social work setting, ideally within community-based services.

*

Strong understanding of safeguarding policies and procedures.

*

Excellent communication, organization, and time management skills.

*

Ability to manage a varied and challenging caseload.
