Pertemps is recruiting for an Occupational Therapist to join a Local Authority reablement team in Chester.As a Reablement Occupational Therapist, you will play a crucial role in supporting individuals to achieve their personal goals and regain independence in daily activities. You will work collaboratively with a multidisciplinary team to deliver person-centred care, assessing, planning, and implementing tailored reablement programs.Key Responsibilities:

• Conduct comprehensive assessments of clients' functional abilities and needs.
• Develop and implement personalized reablement plans.
• Provide therapeutic interventions to enhance clients' physical, cognitive, and emotional well-being.
• Collaborate with healthcare professionals, families, and carers to ensure holistic support.
• Monitor and evaluate progress, making necessary adjustments to intervention plans.
• Educate clients and their families on strategies for maintaining and improving independence.
