A fantastic opportunity has arisen for a Band 6/7 Occupational Therapist to join the Frailty Team in Dorset. This role offers a unique chance to make a significant impact on the lives of elderly patients, helping them maintain their independence and improve their quality of life. The successful candidate will work as part of a multidisciplinary team, providing specialist occupational therapy services to frail older adults with complex health and social care needs.

Location: Dorset
Salary: £25-30 per hour (depending on experience)
Hours: Full-time (open to part-time applicants)

Key Responsibilities:

Conducting comprehensive assessments of frail older adults, identifying functional limitations and developing individualised therapy plans.
Working collaboratively with the Frailty Team to deliver holistic care that promotes independence, safety, and well-being.
Supporting patients with activities of daily living (ADLs), mobility, home safety, and cognitive function.
Providing assistive technology and equipment recommendations to enable patients to live safely at home.
Delivering interventions aimed at reducing hospital admissions, preventing falls, and supporting early discharge from hospital settings.
Ensuring all documentation and patient records are kept up-to-date, including progress notes and discharge summaries.
Offering advice and training to family members and carers to help them support patients at home.
Promoting a patient-centred approach in all aspects of occupational therapy care.
Skills & Qualifications:

HCPC registration as an Occupational Therapist.
Experience working with elderly patients, particularly those with complex medical and frailty needs.
Ability to carry out detailed functional assessments and provide evidence-based interventions.
Excellent communication and interpersonal skills, with the ability to work within a multidisciplinary team.
Strong problem-solving skills and an empathetic approach to patient care.
Experience working in community or hospital settings, with a focus on frailty, falls prevention, and rehabilitation.
This is an exciting opportunity for an Occupational Therapist to work within a supportive and collaborative Frailty Team, offering the chance to make a real difference to the lives of older adults.
