Are you an experienced RMN looking to make a meaningful difference in dementia care? We are seeking a Registered Mental Health Nurse (Band 6) to join our clients multi-disciplinary Memory Assessment Service (MAS) team in Morecambe. This is a fantastic opportunity to work in a specialized role, supporting patients and their carers through high-quality assessment, planning, and care.

Key Responsibilities:

* Serve as an independent specialist practitioner, ensuring effective assessment, planning, consultation, and formulation towards diagnosis within the Memory Assessment Service.

* Demonstrate specialist knowledge and understanding of strategies and interventions for dementia treatment.

* Proficiently refer patients to other agencies within the service or external services as necessary.

* Confidently identify risk factors and take appropriate actions to mitigate them.

* Facilitate clear and effective communication between GPs, LCFT services, and non-statutory services to uphold the highest standards of patient care.

* Provide mentoring and clinical supervision to staff and trainees on placement.

* Actively promote Trust values within the MAS service, aligning with NMC and HPC codes of conduct.

* Support the team manager in providing leadership within the MAS team, ensuring robust assessment, formulation, planning, and monitoring of care for patients and their carers.

Pay Rates:
Days: £22 - £26 per hour
Nights/Saturdays: £30 - £34 per hour
Sundays/Bank Holidays: £38 - £44 per hour

Requirements:
Expertise in dementia care strategies and interventions.
Strong ability to manage risk and adapt care plans accordingly.
Proven skills in multi-agency referrals and collaborative working.
Experience in mentoring and supervising staff or trainees.
Commitment to upholding Trust values and delivering exceptional patient care.
