Job Summary:

We are seeking a dedicated and experienced Registered Mental Health Nurse (RMN) to join our multi-disciplinary Memory Assessment Service (MAS) team based at Altham Meadows/De Vitre House in Morecambe. As an independent specialist practitioner, you will play a crucial role in ensuring the effective assessment, planning, consultation, and formulation towards the diagnosis of dementia within The Bay area.

Key Responsibilities:

Dementia Expertise: Utilize your specialist knowledge and understanding of strategies and interventions in the treatment of dementia to support patients and their carers.

Assessment and Planning: Conduct thorough assessments, develop care plans, and contribute to the accurate formulation towards diagnosis.

*

Interagency Collaboration: Proficiently refer patients to other services or agencies as needed, ensuring a seamless care pathway.

*

Risk Management: Confidently identify risk factors and take appropriate actions to mitigate potential issues.

*

Communication: Facilitate communication between General Practitioners (GPs), other services within LCFT, and non-statutory services to ensure the highest possible standards of patient care.

*

Mentorship and Supervision: Provide mentoring and clinical supervision to other staff members and trainees on placement within the MAS team.

*

Leadership Support: Assist the team manager in ensuring that leadership within the MAS team drives effective assessment, formulation, planning, and monitoring of care provided to patients and their carers.

*

Promotion of Trust Values: Actively promote the Trust's values within the MAS service, adhering to the Nursing and Midwifery Council (NMC) and Health and Care Professions Council (HCPC) code of conduct.

Skills and Experience Required:

* Registered Mental Health Nurse (RMN) with current NMC registration.

* Significant experience in dementia care and memory assessment services.

* Demonstrated ability to work independently and as part of a multi-disciplinary team.

* Strong clinical assessment and risk management skills.

* Excellent communication and interpersonal skills.

* Experience in providing clinical supervision and mentorship.

* Commitment to upholding the highest standards of patient care.
