Job Overview:Become a vital member of the Community Rapid Response Team, delivering urgent, patient-centered occupational therapy services. This role focuses on swift assessments and interventions to prevent hospital admissions and support timely discharges.

Key Responsibilities:

• Conduct urgent assessments for patients in community settings.
• Develop and implement individualized treatment plans to promote independence.
• Collaborate with the multidisciplinary team to coordinate care effectively.
• Support patients with complex needs to avoid hospital admissions.

Key Requirements:

• HCPC registration as an Occupational Therapist.
• Minimum of 6 months NHS experience in community or rapid response settings.
• Strong clinical decision-making and communication skills.
• Ability to work independently in a fast-paced environment.
