Vox Consultants are hiring for an Adult Social Worker to join a London Borough on a Locum basis.

You will be:
Joining the Hospital Discharge Team; your role will be to:

Hold a case loads of service users to facilitate safe and timely discharges, by putting in place the appropriate care plans and packages.
Collaborating with other healthcare professionals to ensure comprehensive care.
Addressing any urgent or complex situations.
Maintaining an accurate records of patient interactions.Essentials:

Registered to Social Work England and has completed an ASYE.
The ideal Candidate will have direct experience of working within a hospital setting within Adult Social Work.
