Location: SloughSalary: £39,513 - £44,711 per annum plus Local Weighting Allowance of £1,064

Key responsibilities:

• Delivering person-centred social work support to individuals and families

• Working closely with multi-disciplinary teams to ensure seamless hospital discharges and continuity of care

• Conducting assessments, reviews, and care planning to promote independence and wellbeing

• Upholding the highest standards of safeguarding and compliance with statutory duties

About you:

• Qualified Social Worker with Social Work England registration

• Experience working in a hospital or locality social work setting

• Strong commitment to equality, diversity, and anti-oppressive practice

• Excellent communication and collaborative working skills
