Salary: £38/hour

Working period: Monday - Friday Hybrid Working

Working Hours: 37 hours per week

Assignment period: 3-6 Months Ongoing

Complex Intervention and Treatment Team

* Conduct comprehensive assessments of social, physical, and mental health needs and develop person-centered care plans in collaboration with service users and professionals.

* Work with healthcare providers and participate in case meetings, ensuring integrated, holistic care through communication with other service providers.

* Provide direct interventions, such as emotional support, crisis intervention, and daily living assistance, while supporting the management of medical and mental health conditions.

* Respond to emergencies, develop crisis prevention strategies, and ensure access to emergency services as needed.

* Regularly review and adjust care plans based on client progress and needs, maintaining accurate case notes and tracking outcomes.

* Act as an advocate for clients' rights, support access to resources, and promote client independence through education and empowerment.

* Provide training on complex care needs and best practices, while staying updated on relevant policies and practices.

* Identify and act on safeguarding concerns, working with safeguarding teams to ensure client safety.

* Complete detailed documentation in line with policies and prepare reports for internal and external reviews.

* Coordinate resources, including services, equipment, and funding, and manage care package budgets while ensuring access to appropriate support services.
