Permanent, Flexible Working Pattern, Up to £23p/h

We are looking for a Nurse that specialises in Long Term Conditions Management in the Sheffield area, to join a trusted practice on a part-time basis, a minimum of 2 days/week. They are offering a great competitive salary, up to £23 per hour, negotiable dependant on experience.

Skills needed:

• Driving License Required (PAID mileage)
• Diploma in Diabetes and/or Respiratory Conditions
• Independent Prescriber (desirable, but not essential)

This role involves carrying out home visit treatment plans including education and lifestyle advice, monitoring and management of symptoms and medications, and insulin management.
