Case Manager

Job Description:

Hours: 40 per week (includes some weekend / evening / Bank holiday hours)

Salary: £26,000 - £29,000 *Plus overtime if required

Reports to: Deputy Manager

Purpose of the Post To be:

A positive role model for the parents, child and practitioners and encourage both to develop meaningful relationships and attachments.

To be responsible for the case management of a defined number of families advising and supporting throughout the assessment process.

Responsible for the safe running of the House on a shift-by-shift basis, ensuring compliance with Residential Family Centre National Minimum Standards, Health and Safety legislation and safeguarding legislation. To participate in the on-call rota

Key Responsibilities

To support the Registered Manager and Deputy to deliver on the content of the Certificate of Registration and the Statement of Purpose of the Centre.

To ensure that the home environment is appropriate and sensitive to the needs of the families and in line with National Minimum Standards for Residential Family Centres 2013.

Support the Registered Manager and Deputy with staff supervision and support and inductions.

Work within the policies and procedures established in the House and those of Resolve Care Ltd and ensuring that all staff are aware of these and adhering to these.

To undertake any additional / alternative duties commensurate with the role.

To participate in the on-call rota.

Case Management

To be responsible for overseeing the management of a defined caseload

To ensure that all documentation is relevant and robust.

To complete the weekly summary report for identified families.

To direct staff on specific key work and traffic light sessions as required and ensure delivery of such to achieve outcomes. August 2023

To liaise with the LA, Guardian and ISWs as necessary and to work with the ISW to agree an assessment plan and implementation of this plan.

To be responsible for ensuring that all meetings are planned and documented appropriately, and that staff are in attendance where necessary.

To direct and oversee co-key workers and other staff who will be working with the family on a daily basis, including supervision of staff where appropriate.

To liaise where necessary with other professionals i.e. health visitors, GPs, social workers.

To assist in preparation for reviews and meetings and attend when required

To develop and sustain collaborative links with a range of agencies and practitioners alongside parents in order to co-ordinate and deliver bespoke support packages to children/young people and their families (e.g. R3).

Promote an assertive but supportive approach with families which engages families in identifying and implementing solutions and ensure that parents/carers and children/young people are able to participate fully in decisions that impact on their wellbeing.

Managing resources

To maintain a safe environment, complying with health and safety and other legal requirements

To maintain the fabric of the building, its grounds and equipment in good order, encourage staff and children to care for their surroundings and make best use of the available resources.

To manage physical, financial and human resources effectively.

To use community resources where appropriate.

Personal

To read and understand Company policies and procedures and ensure their operation within the Centre.

To maintain professional knowledge and skills through reading, attending conferences, training and other such activities, as agreed with the line manager.

To report to a line manager or other appropriate person, in the event of awareness of concerns in respect of families or staff.

Protection of Children and Vulnerable Adults

Health and Safety

August 2023 the post holder will be expected to have an understanding of their responsibility for the health and safety of the staff and families within the Unit.

Technology and Data Protection:

The post holder must be aware of their responsibility for ensuring that all data is collected, stored and shared in line with Data Protection legislation. Due to the nature of the service, the post holder will be expected to carry out any other duties commensurate to the role.
