We are seeking a dedicated and passionate Complex Care Mobilisation Nurse to join a leading and progressive provider of complex care packages within the community, specialising in both adult and paediatric care. This exciting opportunity is based in the Wimbledon area, where the company is experiencing significant growth and has secured new care packages.

As a Complex Care Mobilisation Nurse, you will be pivotal in conducting initial assessments and creating personalised care plans tailored to the unique needs of each client. Working alongside a collaborative team of nurse case managers, you will oversee the on-boarding of new packages, ensuring the highest standard of care is delivered.

Criteria for a Complex Care Mobilisation Nurse:

RGN/RSCN with an active NMC PIN
Proven experience in assessing clients with complex care needs
Expertise in formulating individualised care plans
Recent hands-on experience in tracheostomy care and ventilator management (within the last 2 years)
Exceptional verbal and written communication skills for liaising effectively with patients, families, and the multidisciplinary team (MDT)
Full UK Driving Licence  Key Responsibilities for a Complex Care Mobilisation Nurse:

Develop personalised care plans based on each client's specific needs
Lead and coordinate the MDT, ensuring clear communication and effective teamwork
Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
Deliver nursing interventions, including the management of medication administration
Collaborate closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages  Benefits for a Complex Care Mobilisation Nurse:
