This role is in the Continuing Health Team in Adults.

The role would require in person attendance for completion of assessments and meetings, and attendance in office one day per week.

To complete Care Act Assessments to participate in DST meetings with health professionals. To complete any follow up work to ensure appropriate funding, services and support plans are in place to meet service user needs.
