Salary: £35,992 - £37,536 per annum pro rata + £7,000 Market Rate Supplement (MRS) per annum pro rata
Hours: Full-time (37 hours per week) with part-time and weekend-only options available
Location: Hybrid - Work from home, Central Bedfordshire Council offices (including Priory House, Shefford), and hospitals across Central Bedfordshire

About the Role:

We're seeking dedicated Social Workers, Nurses, or Occupational Therapists to join our dynamic Hospital Discharge Team. This is your opportunity to make a meaningful difference by helping people regain their independence after a hospital stay.

In this hybrid role, you'll play a vital part in ensuring safe and timely discharges, working across hospitals, care homes, and community settings. You'll be empowered to manage your own caseload, collaborating closely with our supportive reablement team to provide the best outcomes for those in need.

Key Responsibilities:

Support individuals through the discharge-to-assess pathway, ensuring they receive the right care and services to regain independence.
Carry out Care Act and Mental Capacity Act assessments in accordance with current legislation.
Work closely with the reablement team to plan safe discharges and improve long-term outcomes.
Be a supportive presence in hospitals, residential homes, and individuals' homes, providing vital guidance at a critical time.

We're looking for:

A proactive team player with a "can-do" attitude.
Someone who can adapt quickly to different situations and settings.
Strong communication skills for liaising with individuals, families, and multi-disciplinary teams.
A Registered Social Worker, Nurse, or Occupational Therapist with the relevant qualification and current registration (Social Work England, Nursing & Midwifery Council, or HCPC).
A full, valid UK driving licence and access to a vehicle.
