Occupational Therapist
Social Care, Reablement
Location: Weston-super-Mare, North Somerset
Department: Adult Social Services
Overview: We are seeking a qualified Occupational Therapist to join the Reablement Intervention team. This role is crucial in supporting adults to regain independence and improve their quality of life through reablement services.
Key Responsibilities: - Assess and identify the needs of individuals referred to the reablement service.
- Develop and implement personalised care plans that focus on reablement and promoting independence.
- Collaborate with multidisciplinary teams, including social care staff, to deliver effective interventions.
- Provide expert advice and support to individuals and their families on adaptive techniques, equipment, and strategies to improve daily living skills.
- Monitor and review progress, adjusting care plans as necessary to achieve desired outcomes.
Requirements: - Qualifications: Degree or equivalent qualification in Occupational Therapy.
- Experience: Previous experience in a social care reablement setting is essential.
- Skills: Strong assessment and care planning skills, excellent communication and teamwork abilities.
- Professional Registration: Must be registered with the HCPC
