Job Description:

We are seeking a motivated and compassionate Band 5 Physiotherapist to join the multidisciplinary team. This is an excellent opportunity for a physiotherapist looking to build on their clinical experience in an inpatient rehabilitation setting, supporting adult patients through their recovery journey.

The role involves working closely with patients who may have cognitive impairments and complex discharge needs, providing safe, effective, and person-centred physiotherapy.

Key Responsibilities:

* Conduct comprehensive physiotherapy assessments, focusing on mobility and transfer risk assessments.

* Set and review individualised, meaningful goals with patients to support their rehabilitation progress.

* Deliver safe and effective physiotherapy interventions and monitor the need for pain control around exercise sessions

* Collaborate with the Multidisciplinary Team (MDT) to plan and implement holistic patient care and discharge planning.

* Communicate clearly and confidently both verbally and in written format, including handovers and documentation.

* Support and guide therapy assistants, helping them prioritise and plan their daily activities effectively.

* Gather and share relevant information to aid safe and timely discharge from hospital.

* Demonstrate empathy, patience, and understanding for patients who may be in unfamiliar environments and experiencing cognitive challenges.

Essential Requirements:

* HCPC-registered Physiotherapist.

* Strong understanding of risk assessment in relation to mobility and transfers.

* Excellent interpersonal and communication skills.

* Ability to work independently and as part of a collaborative MDT.

* A compassionate approach to patient care, especially with those who may have cognitive impairments or emotional vulnerabilities.

* Good organisational and time-management skills
