Care Manager | Hammersmith, West London | 40 hours per week, 9:00 am - 5:30 pm, Monday to Friday plus one weekend per month on call | £34,000 - £37,000 PA | 35 days annual leave

As a Care Manager, you will play a pivotal role in ensuring the rights, needs and preferences of clients are always at the forefront. You will line manage a diverse staff team, providing mentorship, supervision and pastoral support to ensure the organisation's values are consistently put into action.

This is an office-based role (office in Hammersmith) with the flexibility to work at home for one - two days per week. Candidates should reside locally in West London and be able to travel with a bike or car in order to visits clients homes in Ealing, Chiswick, Hammersmith and Fulham. 

Are you the right person for the job?

Strong people management experience within domiciliary care or similar sectors
Proven track record in supervising and developing health or social care staff
Experience in building positive relationships with clients, relatives and loved ones 
Excellent listening, written, and verbal communication skills
Strong organisational and prioritisation abilities
Positive and empowering attitude towards older people and those with social care needs
Commitment to the values of Compassion, Integrity and Respect
Honest, trustworthy, reliable and able to work under pressure
Physically and mentally capable of fulfilling the role
Enhanced Disclosure and Barring check
NVQ / Diploma Level 5 qualification or equivalent in a care/health-related area
Full driving licence and use of a car or bike
What will your role look like?

Assessments and Care Planning

Conduct comprehensive risk assessments and develop individualised Care Plans
Maintain regular contact with clients and their families to ensure satisfaction
Collaborate with family members and agencies for integrated care services
Maintain accurate and timely records and ensure adherence to relevant legislation
Team management 

Ensure a safe and supportive environment for care staff
Oversee the induction, supervision, and appraisal of staff
Develop and deliver training programs and support professional development
Address any performance issues promptly and effectively
Compliance and Quality Assurance

Ensure compliance with the Care Quality Commission and other quality standards
Identify and resolve instances of non-compliance
Contribute to company development and review cycles
Other Responsibilities

Participate in legislative assessments and company development
Ensure budget adherence and accurate record-keeping
Foster strong communication and working relationships across the company
For an additional monthly payment cover on-call from 5pm Friday - 9am Monday, one weekend per month

Stage two - Person to person interview with the Registered Manager, Director, and the HR Manager
