Job Overview:We are seeking a skilled Band 6 Occupational Therapist to join our team, providing comprehensive assessment, treatment, and discharge planning for patients in acute general medicine, frailty wards, and front door services.

Key Responsibilities:

• Conduct in-depth occupational therapy assessments and develop individualized treatment plans.
• Facilitate safe and effective discharge plans tailored to patient needs.
• Collaborate with multidisciplinary teams to support patient-centered care on acute and frailty wards.

Requirements:

• HCPC registration and NHS experience as a Band 6 Occupational Therapist.
• Strong clinical skills in assessment and treatment planning for acute and frail patient populations.
• Excellent communication and teamwork abilities.
