About the Role:
Are you passionate about supporting individuals living with dementia? Woodlands Care Home is seeking a Dementia Coordinator to provide compassionate care and guidance to residents and their families. This role focuses on person-centred support, helping residents live well with dementia, while empowering them to engage in meaningful daily activities.
Key Responsibilities:
- Collaborate with residents and their families throughout the moving-in process, ensuring a smooth transition and a welcoming environment.
- Develop tailored, person-centred support plans reflecting the individual needs, preferences, and interests of residents.
- Design and coordinate well-being programmes and activities that align with residents' hobbies and routines, enhancing their daily experience.
- Liaise with families to identify and implement effective strategies for managing dementia-related behaviours.
- Support staff in building dementia care skills, completing documentation, and responding to stress or care refusal situations.
- Organise community events, including quarterly dementia cafés, to foster peer support and share best practices.
- Work closely with allied health professionals to improve residents' quality of life and help reduce reliance on psychoactive medications.
- Regularly review and assess residents' needs, ensuring ongoing, high-quality care.
