Occupational Therapist

This role is for an Occupational Therapist- involves carrying out Care Act assessment for those discharged from hospital to a step down bed, which could be within a community rehab but primarily a nursing home setting. The post holder will need work with MDT professionals to explore how best to support the person to return to their home in the community and arrange any care and support to facilitate this. On occasion this may include completing CHC assessments and mental capacity assessments as well as functional assessments.
