An exciting opportunity has arisen for a Band 7-8a Locum Pharmacist to join our team in Aylesbury. We are looking for an experienced and detail-oriented professional to play a key role in ensuring safe and effective medication management for patients.
Key Responsibilities: - Clinically validate medication charts and discharge prescriptions, ensuring safety and appropriateness.
- Accurately check dispensed prescriptions to maintain the highest standards of patient care.
- Complete medicines reconciliation to ensure accurate medication histories.
- Review and check Patients Own Drugs (PODs) for safety and compliance.
- Provide expert advice on medication management to healthcare professionals and patients.
Essential Requirements: - GPhC registration as a qualified Pharmacist.
- Proven experience in a hospital or similar clinical setting.
- Strong knowledge of medicines reconciliation and prescription validation.
- Attention to detail with excellent organizational and communication skills.
