Job Introduction
At Oxfordshire County Council, we empower you to grow, learn, and thrive. Whether you're advancing your career in adult social care or seeking new challenges, we provide the support to help you succeed.

Our vision is to deliver a person-centred, strength-based approach to care, keeping people healthy, independent, and in their communities for as long as possible.

About the Role
We are transforming our hospital discharge services to help Oxfordshire residents have their needs met in their own homes rather than in hospital. As part of our Home First D2A team, you will:

Support individuals to regain independence through a strength-based approach, working closely with reablement providers and the voluntary sector.
Assist with goal planning for short-term care after hospital discharge or to prevent hospital admission.
Use your expertise in activity analysis, assistive technology, and minor adaptations to reduce formal care.
Apply moving and handling skills as part of your role.About You
You are:

Committed to continuous learning, supported with development opportunities.
Driven to find creative solutions and work collaboratively with individuals, rather than for them.
A self-leader with a passion for making a difference in adult social care.
Compassionate, respectful of diverse backgrounds, and advocate for equality.
Skilled in communication and collaboration to achieve the best outcomes.
A valid driver's licence holder with access to a vehicle.Rewards and Benefits
