An excellent opportunity has arisen for a Band 6 Occupational Therapist to join a dedicated team working within a community setting.

Setting: Community

Location: Milton Keynes
Position: Full-Time (Part-Time Considered)
Hourly Rate: £25 - £27 per hour
Start Date: ASAP

Key Responsibilities:

Conduct comprehensive assessments and provide tailored interventions for a diverse caseload of patients within the community.
Develop, implement, and review care plans that aim to enhance patients' independence and improve their quality of life.
Work closely with multidisciplinary teams, including social workers, GPs, and district nurses, to provide holistic care.
Support patients with rehabilitation and long-term conditions, focusing on maximizing their functional abilities and promoting community integration.
Ensure timely documentation, including patient progress reports, assessments, and discharge plans.
Provide education and support to patients, their families, and carers, helping them to manage ongoing care needs in the community.
Required Skills & Experience:

HCPC registration as an Occupational Therapist.
Prior experience working in a community setting or with a strong understanding of community-based care.
Ability to work independently while contributing to a multi-disciplinary team.
Excellent clinical reasoning, assessment, and communication skills.
Experience with a wide range of patient conditions and a commitment to providing person-centred care.
Flexibility to work full-time or part-time, depending on individual preferences.
