Job Summary:

An opportunity is available for a dedicated Band 7 Multidisciplinary Pathway Worker to support our team in Kent (Location:CT10 2TT). This role focuses on mental health assessments, clinical harm reviews, and reducing wait times for young individuals in need of mental health services. We are seeking a professional with advanced clinical skills and a collaborative approach to provide high-quality, timely mental health care.

Key Responsibilities:

* Duty Coverage: Offer continuous support on-site, addressing urgent cases and inquiries as part of routine duty responsibilities.

* Initial Clinical Assessments: Conduct thorough clinical assessments using the biopsychosocial model to evaluate mental, physical, and social factors influencing young people's health.

* Waiting List Management: Proactively manage waiting lists, triaging cases based on clinical need and urgency to ensure prompt service delivery.

* Clinical Harm Reviews: Conduct comprehensive harm reviews for young people awaiting care, prioritizing high-risk cases for immediate intervention.

* Multidisciplinary Care Coordination: Work collaboratively with team members across psychiatry, psychology, nursing, and social work disciplines to ensure integrated care.

* Case Documentation: Accurately maintain and update patient records on electronic health systems (e.g., EMIS, Rio), ensuring data security and compliance with confidentiality standards.

* Risk Assessment and Crisis Management: Perform and document risk assessments using validated tools and implement risk management plans, responding effectively to crises as they arise.

Essential Skills and Technical Requirements:

* Band 7 Qualification: Previous experience in a Band 7 role within mental health services, specifically in child and adolescent mental health.

* Professional Registration: Valid NMC (Nursing and Midwifery Council) or HCPC (Health and Care Professions Council) registration is mandatory.

* Enhanced DBS Clearance: A recent Enhanced Disclosure and Barring Service (DBS) check is required, or willingness to undergo one if not current.

* Experience with Mental Health Assessments: Skilled in using child and adolescent mental health assessment tools such as the Strengths and Difficulties Questionnaire (SDQ), Health of the Nation Outcome Scales for Children and Adolescents (HoNOSCA), or similar.

* Advanced Communication and Counselling Skills: Competency in motivational interviewing, active listening, and family-centered communication to support young people and their families.

* Proficiency with IT Systems: Competent in using digital health systems (e.g., EMIS, Rio) for case management, record keeping, and service coordination.

* Crisis and Risk Management Expertise: Experience in conducting crisis assessments and managing risks effectively for high-risk individuals, including developing and implementing safety plans.
