We are looking for a passionate Complex Care Mobilisation Nurse to join a well-established and progressive provider of complex care packages within the community, specialising in adult and paediatric complex care. This role will be based in the Wimbledon area where the company has secured further packages during an exciting growth phase.

The successful Complex Care Mobilisation Nurse will be responsible for conducting initial assessments, whilst creating personalised care plans for each person. You will work closely with a core team of nurse case managers, on-boarding the packages.

Criteria for a Complex Care Mobilisation Nurse:

RGN/RSCN with an active NMC PIN
Experience in assessing clients with complex care needs
Experience in formulating individualised care plans   
Up to date experience of tracheostomy care and ventilators within the last 2 years
Advanced communication skills verbally and written to liaise effectively with patients, families and the MDT
Full UK Driving Licence 
Key responsibilities for a Complex Care Mobilisation Nurse:
 
Design a tailored care plan specific to an individual client's needs
Effectively manage the MDT through strong communication and teamwork
To provide ongoing monitoring, support and assessment of client's physical and physiological needs
To deliver nursing interventions including the management of medicine delivery
Working closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care
Benefit package for a Complex Care Mobilisation Nurse:
 
Up to £50,000 per annum DOE
Office based and community working 
Monday to Friday (9-5) hours
33 days holiday
