Salary - Up to £41,500 DOE
Location: London - West / North / Central (Hybrid)

Purpose
The Client's programme, a government-funded initiative, provides early-intervention work and health support to overcome health-related barriers to employment. The programme works with participants who are currently employed but facing difficulties in the workplace, as well as those who have fallen out of work and need assistance to return to employment. The role involves engaging with both participants and employers to ensure mutual agreement on the way forward.

Main Duties and Responsibilities

Coordinate and support the delivery of individual Health and Work Action Plans.

Partner with employers to provide practical advice on supporting employees with health conditions, including workplace adjustments.

Provide wellbeing advice to promote healthy lifestyle changes.

Offer employment coaching, including careers guidance, CV building, and job brokerage.

Build and maintain strong relationships with referral partners, including GPs and Community Health Teams.

Promote the service to employers to meet programme entry targets.

Collaborate with a Multi-Disciplinary Team (MDT) including Occupational Therapy and Mental Health Practitioners.

Manage a diverse caseload of clients.

Conduct holistic assessments with programme participants.

Co-produce tailored action plans with participants.

Maintain a network of referral partners for specialist support.

Provide in-work support to increase the likelihood of participants remaining employed.

Maintain accurate and compliant electronic client records.

Adhere to The Client's safeguarding policies and promote a safe working environment.

Maintain confidentiality and comply with Data Protection regulations.

Promote and implement The Client's Risk, Quality, and Information Security Management Systems.

Report any risks or concerns to the appropriate person.

Support The Client's strategic aim to adopt green approaches and become carbon neutral.
