We are seeking a dedicated and experienced Advanced Nurse Practitioner to join our dynamic team within a primary care setting in Liverpool. Day-to-day of the role: - Conduct comprehensive patient assessments and develop individualised treatment plans.
- Provide expert clinical advice and support to patients, managing both chronic conditions and acute illnesses.
- Prescribe medications, utilising the V300 qualification.
- Collaborate with healthcare professionals to ensure integrated and cohesive care delivery.
- Maintain up-to-date patient records and participate in continuous professional development.
Required Skills & Qualifications: - Master’s degree in Advanced Practice.
- V300 Prescribing qualification.
- Proven experience in primary or urgent care settings.
- Strong clinical skills with a patient-focused approach to care.
- Excellent communication and teamwork abilities.
