Schedule: 9-5 Monday to Friday 
Start Date: ASAP
Rate of pay: £26-30 p/h umbrella days
Must holder a Valid Full UK Drivers License
Position Overview: We are seeking a skilled and compassionate Forensic Social Worker to provide specialized care to individuals with mental health needs within the forensic setting. The ideal candidate will have a strong background in mental health nursing and a commitment to promoting recovery and rehabilitation for individuals with complex mental health issues.

You will contribute fully to risk assessments and care pathway planning. You will work to the principles of the Recovery Model and will provide a social care perspective to patient care planning. You will have an active role in working closely and supporting the families and Carers of patients. You will take a lead on ensuring safeguarding adult and children concerns are progressed, working closely with the Team Manager.

Key Responsibilities:

Provide comprehensive nursing assessments and interventions for individuals within the forensic mental health system, including those in correctional facilities and forensic psychiatric hospitals.
Collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to the unique needs of each patient.
Conduct therapeutic interventions and facilitate group therapy sessions to support patients in developing coping skills and addressing underlying mental health issues.
Coordinate discharge planning and transition services to facilitate successful reintegration into the community.
Maintain accurate and up-to-date clinical documentation in accordance with professional standards and regulatory requirements.
Requirements and qualifications required for this role:

Current and valid registration with the NMC.
Proven experience as a social worker.
Strong communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
