The Care Coordinator is responsible for managing and facilitating patient care by working closely with healthcare teams, patients, and their families to ensure seamless delivery of services. The Care Coordinator will be involved in assessing patient needs, developing care plans, ensuring effective communication between parties, and monitoring the patient's progress.

Key Responsibilities:

Assessment & Planning:

Conduct initial and ongoing assessments to determine the care needs of patients.
Develop personalized care plans that address the patient's medical, physical, and emotional needs.
Coordinate services across multiple providers to ensure a holistic approach to care.
Care Coordination:

Liaise with doctors, nurses, specialists, and social workers to ensure the patient receives comprehensive care.
Schedule and facilitate appointments, treatments, and home visits for patients.
Provide ongoing communication with patients and their families regarding care updates, changes, and instructions.
Monitoring & Evaluation:

Regularly monitor patient progress and make adjustments to care plans as necessary.
Evaluate patient outcomes and ensure care is in line with clinical guidelines and best practices.
Support & Advocacy:

Act as the primary point of contact for patients and families, offering support and education on the care process.
Advocate for patients in healthcare settings, ensuring they have access to the necessary resources.
Help patients understand their treatment options, insurance coverage, and rights.
Documentation & Reporting:

Maintain accurate records of patient interactions, care plans, and medical history.
Prepare reports for healthcare teams and management on patient progress and needs.
Ensure compliance with healthcare regulations, patient privacy laws, and organizational policies.
Collaboration:

Work in collaboration with other team members, such as social workers, therapists, and physicians, to ensure high-quality patient care.
Participate in team meetings to discuss patient care progress and share insights.
