Job Description:

We are currently seeking a dedicated and compassionate Community Nurse to join our team on an agency basis in Swindon. You will provide high-quality care to patients within their homes and community settings, supporting them with healthcare needs, recovery, and independence.

Key Responsibilities:
- Deliver professional, patient-centered nursing care to individuals in the community.
- Assess, plan, and implement care plans in collaboration with the multi-disciplinary team.
- Administer medications, dress wounds, and monitor vital signs.
- Provide health education to patients and their families to promote wellness and manage illness.
- Manage long-term conditions, palliative care, and post-hospitalization recovery.
- Maintain accurate patient records and ensure continuity of care.

Requirements:
- Valid NMC registration (Nursing and Midwifery Council)
- Experience in community nursing or similar roles (minimum 6 months preferred)
- Excellent communication and organizational skills
- Ability to work autonomously and manage time effectively
- Full UK driving license and access to a vehicle
