As a Team Lead Nurse, you'll play a vital role in guiding a team of skilled nurses and healthcare assistants, ensuring exceptional care for patients in their homes. Your Responsibilities:
* Lead and Inspire: Provide clinical leadership and day-to-day management of a team of nurses, healthcare assistants, and associate nurses, ensuring high standards of care and effective teamwork.
* Patient-Centered Care: Take responsibility for managing a caseload and delivering evidence-based nursing care, focusing on health needs assessments, complex care plans, and coordinated discharge planning to prevent hospital admissions.
* Collaboration Across Services: Work closely with GPs, social care, and other health and voluntary services to provide seamless, integrated care tailored to each patient's unique needs.
* Drive Quality and Improvement: Promote service development by monitoring care quality, identifying opportunities for improvement, and supporting continuous professional development within the team.
* Mentorship and Support: Offer practical teaching and mentorship, overseeing clinical competency and providing guidance to junior staff and students.
Who We're Looking For:
* Qualifications: Registered General Nurse with current UK NMC registration and a Specialist Practitioner qualification in District Nursing (or working towards it).
* Experience: Proven experience in community nursing with a focus on complex care, team coordination, and multidisciplinary collaboration.
* Skills and Attributes: Strong leadership skills with the ability to motivate, delegate, and handle complex care decisions. Effective communication, problem-solving abilities, and a commitment to continuous learning are essential.
Person Specification:
Education/Qualification
* Registered Nurse with current UK NMC registration
* Evidence of continuing professional education and training at degree level
* Qualification in team / service management or prepared to work towards.
* Mentor or practice-based learning training.
* Community or Independent Nurse Prescribing qualification or prepared to work towards
* Post - registration qualification in district nursing or equivalent (desirable)
Experience
* Evidence of recent post-registration nursing experience in community nursing
* Experience of negotiating and liaising within a multi-disciplinary team
* Experience of working with people with complex needs
* Experience of co-ordinating and managing a team
* Experience of teaching others and mentoring students
* Experience of evidence-based care
* Experience of undertaking comprehensive health needs assessment and developing complex care plans
* Experience of co-ordinating and delivering care for people with complex, long-term conditions.
