Job Overview:We are seeking a dedicated and skilled Band 6 Physiotherapist to join our team, focusing on supporting patient flow and providing high-quality care in a fast-paced environment. The role involves conducting same-day assessments and facilitating timely discharges within the Frailty Same Day Emergency Care (SDEC) setting. You will play a crucial role in enabling rehabilitation and re-enablement to ensure effective discharge planning and improved patient outcomes.

Key Responsibilities:

  • Perform comprehensive physiotherapy assessments for patients within the Frailty SDEC unit.
  • Plan and deliver tailored rehabilitation and re-enablement programs to meet individual patient needs.
  • Facilitate same-day discharges by coordinating with multi-disciplinary teams and implementing effective discharge plans.
  • Support patient flow by ensuring timely and appropriate interventions that align with care pathways.
  • Provide expert advice to patients, families, and caregivers to optimize recovery and promote independence.
  • Maintain accurate clinical documentation and contribute to the development of service improvement initiatives.

Requirements:

  • HCPC registration as a Physiotherapist.
  • Proven experience in acute or frailty care settings, with a focus on rehabilitation and discharge planning.
  • Strong clinical assessment and intervention skills.
  • Excellent communication and collaboration abilities to work effectively within a multi-disciplinary team.
  • Ability to manage a dynamic caseload and adapt to a fast-paced environment.
