Role Overview:

We are seeking a highly organized and compassionate Care Coordinator to join our team. The ideal candidate will have excellent communication and time-management skills, along with a strong understanding of the care industry. As a Care Coordinator, you will play a key role in coordinating and managing the care services for our clients, ensuring that their needs are met and their care plans are delivered effectively.

Key Responsibilities:

Coordinate and manage the care services for our clients, ensuring that their needs are met and their care plans are delivered effectively
Liaise with clients, their families, and care staff to ensure that all care needs are met and any concerns are addressed promptly
Conduct assessments and develop personalized care plans for clients, taking into consideration their individual needs and preferences
Monitor and review care plans regularly to ensure they are up-to-date and reflective of the client's changing needs
Schedule and coordinate care staff rotas to ensure adequate coverage and continuity of care for clients
Provide support and guidance to care staff, ensuring they have the necessary training and resources to deliver high-quality care
Maintain accurate and up-to-date client records and ensure confidentiality is maintained at all times
Participate in on-call duties as required, providing support and guidance to care staff and clients outside of regular office hoursRequirements:

Minimum of 2 years of experience in a similar role, preferably within a domiciliary care setting
NVQ Level 3 in Health and Social Care or equivalent qualification
Excellent communication and interpersonal skills
Strong organizational and time-management skills
Knowledge of care industry regulations and standards
Ability to work well under pressure and handle multiple tasks simultaneously
Proficient in using computer systems and software, including Microsoft Office
