Location: Gravesend, Kent
Pay Rate: Up to £38 p/h

Job Description:

We are looking for a skilled Social Worker to join our Community Social Work Team (CSWT) in Gravesend, Kent. This role will require the ability to assess and manage a caseload of individuals with a variety of needs.

Key Responsibilities:

* Assess the needs of vulnerable individuals and families in the community.

* Develop care plans and strategies for service users to achieve positive outcomes.

* Collaborate with local agencies and professionals to deliver holistic care.

* Ensure compliance with safeguarding and risk management policies.

* Maintain accurate case records and documentation.

Qualifications & Requirements:

* Essential: Social Work qualification (e.g., Degree in Social Work, Postgraduate Diploma in Social Work)

* Essential: Registration with Social Work England (SWE).

* Experience working in a community-based social work setting.

* Ability to work under pressure and manage a busy caseload.

* Strong communication and interpersonal skills.
