We are looking for someone who can work both autonomously and within a wider team who will be predominantly based (80% of the time) in Chertsey. The role in the main will be from 8AM to 4PM but there will be some flexibility that will be needed to complete the role effectively. The same applies to the working days, whilst it is mostly Monday to Friday, this may not always be the case as the primary role is to assess people who are medically fit for discharge to be admitted in to one of our care homes nearby.
