We are seeking a dedicated and compassionate Advanced Practitioner for our Leaving Care team. This role is crucial in supporting young people transitioning out of the care system, helping them achieve independence and build a brighter future.
Day-to-day of the role: - Conduct thorough assessments of young people’s needs and develop tailored care plans to support their journey to independent living.
- Provide emotional and practical support, advocating for young people’s rights and ensuring they have access to essential services such as housing, education, and employment.
- Collaborate with various agencies, including social services, healthcare providers, and educational institutions, to coordinate comprehensive support and resources.
- Offer guidance and mentorship to help young people develop life skills, build confidence, and make informed decisions about their future.
- Regularly monitor and review the progress of care plans, making necessary adjustments to ensure ongoing support and positive outcomes.
Required Skills & Qualifications: - Diploma or degree in social work, CQSW, CSS, or equivalent.
- Registered with Social Work England (SWE).
- UK-based statutory social work experience.
- Must have passed the Assessed and Supported Year in Employment (ASYE) if qualified after 2012.
- A full driving licence and willingness to drive.
- DBS clearance.
- Experience at Senior Social Worker level.
