Job OverviewWe are seeking a dedicated Band 5 Occupational Therapist to join the Older People's team. This role involves working closely with patients to improve their quality of life, promoting independence, and supporting recovery.

Responsibilities

• Assess, plan, and deliver occupational therapy interventions for older people.
• Work collaboratively within a multidisciplinary team to create tailored care plans.
• Conduct home assessments and recommend necessary adaptations.
• Provide advice and support to patients, families, and caregivers.
• Maintain accurate patient records and ensure compliance with NHS standards.

Requirements

• HCPC registration as an Occupational Therapist.
• Previous experience in older people's care is preferred but not essential.
• Strong communication, organizational, and problem-solving skills.
• Ability to manage a diverse caseload independently.
