We are looking for someone that enjoys being part of an integrated approach in working with discharge planning from Hospital.  They should have a good knowledge of Continuing Health Care and the assessment process and working within the Discharge to Assess model. Care Act Assessments, Mental Capacity, Best interest decisions and safeguarding all form part of the work that is supported within the team. We have a permanent duty role in place so this isn't generally required as part of the post.This is an agency post and Pertemps can offer you:
