We are seeking a dedicated and professional Social Care Assessor to join an established Adult Services & Health team at Liverpool City Council. 35 hours per week (Monday to Friday, 9:00 AM - 5:30 PM)
1 month initial contract with possibility of extension after this
£21.00 LTD per hour inclusive of holiday payResponsibilities:

Assess the care and support needs of adults and their families, ensuring compliance with statutory frameworks.
Work collaboratively with health professionals and other agencies to develop and implement care plans tailored to individual needs.
Provide guidance to clients on accessing appropriate support and services.
Maintain accurate and up-to-date records in line with council policies and procedures.
Ensure all actions are in line with safeguarding principles and promote the independence and well-being of clients.Requirements:

Previous experience in a social care role, particularly within adult services.
Strong understanding of relevant legislation, policies, and procedures in social care.
Excellent communication and interpersonal skills to work effectively with clients, families, and multi-disciplinary teams.
Ability to manage a diverse and demanding workload, prioritising tasks efficiently.
A commitment to promoting equality and diversity in service delivery.
