About the Role

You'll be joining a proactive and supportive Access and Wellbeing Team focused on prevention, enablement, and strength-based support. This includes medium to longer-term social work interventions, safeguarding enquiries, reassessments, and reviews.

🔹 Must be available to be physically present in the office 3 days per week
🔹 Role includes:

Home visits 🏠
Joint working with other professionals 🤝
Duty responsibilities 📞✅ About You:

Qualified and progressed Social Worker, registered with Social Work England.
Completing Care Act assessments 📋
Court of Protection and Human Rights assessments ⚖️
