Key Responsibilities:
- Conduct comprehensive patient assessments, completing a minimum of four Decision Support Tools (DSTs) per week.
- Engage directly with patients during face-to-face assessments across the Birmingham and Solihull (BSOL) area.
- Undertake report writing and administrative duties remotely as required.
Candidate Requirements:
- Experience: Recent and demonstrable CHC assessment experience is essential.
- Availability: Willingness and ability to travel within the BSOL area for patient-facing assessments.
- Working Style: Strong organisational skills to balance fieldwork with administrative tasks.
