Hospital Liaison Senior Recovery Coordinator

Location: STARS Southend

Salary: £26,993 per annum

Job Type: Fixed Term Contract

As directed by the Team Leader, Lead Nurse and Service Manager, the post-holder will be an integral member of a multi-disciplinary team. They will undertake duties involved in the day-to-day operation, management and development of drug and alcohol treatment in Southend and will be responsible for setting up pathways and delivering the full range of services in Southend Hospital.

The post-holder will also work in Partnership with key partners and the wider integrated treatment team to identify the 'frequent flyer’ cohort and ensure that these individuals are proactively provided with in-reach services upon admission into Hospital.

The post holder will coordinate regular multi-agency professionals, meetings with representatives from relevant Hospital departments, mental health, safeguarding and other involved agencies in order to further identify and implement personalised strategies for supporting particularly vulnerable service users into treatment whilst also reducing the overall rate of hospital admission.

The post-holder will provide alcohol harm reduction advice, brief intervention, support Hospital personnel in setting up three-way discharge planning, and provide formal and informal training on brief intervention to Hospital personnel.

Knowledge, Skills and Experience

  • Experience of working within substance misuse treatment services
  • Demonstrable experience of effective partnership working and relationship management
  • Experience of project development and management
  • Sound and demonstrable knowledge of harm reduction and health promotion interventions
  • Knowledge of the issues facing substance misusers including appropriate medical interventions, social care and health issues
  • Experience of managing a case load and achieving positive outcomes for service users
  • Access to own transport
  • Knowledge of local services and geography
  • Motivational interviewing techniques
  • Qualification(s) in substance misuse
  • Previous experience of working in a Hospital environment
