We are seeking a passionate Nurse lead to join a well-established and progressive provider of complex community care packages. As a leader in specialised complex care, we are expanding our presence in Yorkshire and need a dedicated professional to help us grow.

What We're Looking For:

• Registered General Nurse (RGN) with an active NMC PIN.
• Experience with PEG feeds, tracheostomy care, and ventilators (within the last year).
• Strong leadership and management skills, with the ability to inspire others.
• Advanced written and verbal communication skills for effective liaison with clients, families, and multidisciplinary teams.
• Full UK driving licence and access to a vehicle.
• Ideal for Band 5/Band 6 nurses with a passion for leading healthcare assistants.

Key Responsibilities:

• Carry out comprehensive risk assessment and tailored care plans.
• Monitor, support, and assess clients' physical and physiological needs.
• Lead a multidisciplinary team (MDT) with strong communication and teamwork.
• Deliver nursing interventions, including medicine management.
• Build and maintain meaningful relationships with clients and their families.
• Collaborate with community teams, CHC nurses, and CCG commissioners to maintain bespoke care packages.

Benefits Package:

• Salary: Up to £45,000 per annum (DOE).
• Hours: 40 hours/week, 5 days/week.
• Holidays: 33 days (including bank holidays).
