Are you an experienced Adults Social Worker looking for a rewarding role in a fast-paced environment? We are seeking a dedicated professional to join Swindon's Initial Contact Team, providing vital first-line support and safeguarding adults at risk.

Role & Responsibilities:

* Triage and assess incoming referrals for adults requiring social care support.

* Conduct initial risk assessments to determine the level of intervention needed.

* Work collaboratively with multi-agency teams, including health professionals and community services.

* Provide advice, signposting, and crisis intervention where required.

* Maintain accurate case records and reports, ensuring compliance with statutory guidelines.

Requirements:

✔️ Qualified Social Worker (Degree or equivalent).
✔️ Registered with Social Work England.
✔️ Experience in adult safeguarding, assessments, and care planning.
✔️ Strong knowledge of Care Act 2014, Mental Capacity Act, and relevant social care legislation.
✔️ Excellent decision-making, communication, and crisis management skills.
