Education / Qualifications

* Registered Nurse - degree level or equivalent

Experience & Abilities

* Knowledge of National Framework for NHS Continuing healthcare and Nursing Funded Care (Oct 2018 revised) and associated legislation and guidance

* 3 years post registration experience

* Experience of establishing & maintaining good working relationships

* Experience of imparting unwelcome news to patients and their carers for example regarding their prognosis

Skills and Abilities

* Problem solving skills

* Excellent negotiating skills (to promote change & when dealing with resistance to change)

* Ability to assess patients with complex needs

* Understanding of implications of working across the NHS and the independent sector

* Ability to coordinate case conferences/MDT meetings

Personal Qualities

* Assertive & sensitive to other people's feelings and attitudes - empathic

* Awareness of own development needs

* Commitment to collaborative working

* Need to be a successful team player & enjoys working as part of a team
