We are looking for a passionate individual who is determined to ensure that all Looked After Children and Care Leavers receive the best opportunities, support, and pathways to successful independence. Your primary goal will be to help our care leaver's transition to adulthood through practical and well-planned interventions. Through practical and planned interventions, you will care leavers make a successful transition to adulthood. Personal Advisors hold a statutory function, working with care leavers (aged 18 to 25).

You will befriend, advise, assist, to enable care leavers meet their goals by providing direct practical support, working within multidisciplinary networks within the statutory and non-profit sectors. We are committed to increasing the number of care leavers in education, employment, and training. We also aim to safeguarding young adults and ensure good accommodation outcomes.

To succeed in this role, you will be: Organised and committed to working in co production with our young people, determined to deliver the services they deserve. Passionate about making a positive impact on care leavers' lives, working with our young people to set clear and SMART goals in their Pathway Plans and helping them achieve their ambitions. An excellent communicator with strong interpersonal and influencing skills, able to collaborate effectively with our young people. Ability to record in a timely way with a first person approach.

Responsibilities

1. Work directly and in partnership with the young person to provide a clear and accurate assessment of the young person and their needs, in line with statutory guidance. Undertake interviews, observations, and gather information from young people, carers, family and friends, and other professionals as directed by the Team Manager.

2. On the basis of assessed strengths, needs and risks, devise, implement and review pathway plans, collaboratively with the young person.

3. Manage the caseload as directed, and organise and plan work activities, prioritising tasks and responsibilities.

4. Provide written and verbal reports, which are concise, informative and based on analysis of complex evidence, for a range of audiences, including young people, reviews, panels and other formal meetings, and court..

5. Work with key professionals from the department and externally to offer advice and implement pathway plans. Contribute to professional discussion and challenge in an appropriate manner.
